You may also pick up transportation forms at the District Office Building located at 321 List Ave. Please allow 3 – 5 business days for processing. All forms are processed in the order they are received.
| Transportation to and from regular school sessions is provided for resident pupils who live within the following mileage limits as measured between school and home using our transportation software by the nearest available route (public highway): |

Private/Parochial Required Information Form – Due April 1
If your child currently attends or plans to attend a private/parochial school, the Private/Parochial Required Information Form must be renewed each school year regardless of your transportation needs. New York State law sets April 1st as the deadline for this form. Please be sure to review the Transportation Eligibility Guidelines provided above. If you are requesting transportation and forms are received after the April 1st deadline, transportation may be provided if there is no additional cost to the district.

Transportation to a Childcare/Alternate Site Form – Due April 1
If your child will be utilizing a childcare provider, the Transport to Childcare Form must be submitted each school year no later than June 1st. Please keep in mind you must meet the districts mileage requirements in order to receive transportation to and/or from your student’s childcare provider (refer to the chart above). Should your form be submitted after the June 1st deadline, there may be a delay in accurate scheduling.

Opt-Out of partial/full busing needs Form
In an effort to improve transportation efficiency and therefore reduce the student ride times we are requesting parents who do not wish to use district transportation to let us know by completing this form. By knowing in advance that a student is not riding the bus, it will allow us to create routes that are more accurate by not having unnecessary stops; therefore assist us in getting students to and from school in a more timely fashion. If during the school year your transportation needs change, you can always opt back in.

K-3 Transportation Release Authorization Form
This form will allow your student to be released at the bus stop without a parent/guardian present. If no form is received, for the safety of your children, WICSD will continue to reinforce the district rule that no child in grades K through 3 will be let off the bus without a parent or guardian visibly present at the bus stop. If a parent/guardian is not visible from the bus stop, the child will remain on the bus and considered a lock out
